Registration

Forms and Information

All dogs must have a completed application on file with Dog Tired.  Paperwork should include a complete pet personality profile, owner contact form, signed daycare contract and proof of current vaccinations from a licensed veterinarian.

As enrollment for Spring 2007 has been closed due to the large number

of dogs currently attending daycare, please be sure to submit a wait list request in conjunction with these registration forms.  You can submit a wait list request by clicking on the wait list button to the left.
